Your Feedback Matters
Feedback helps Connect understand what is working well, where experiences could be improved and how we can continue providing quality services across Karratha and the Pilbara.
We aim to make it straightforward to speak up. You will be treated respectfully when sharing feedback or making a complaint, and raising a concern will not affect your right to access services.
Compliments
Tell us about a positive experience or recognise a team member who made a difference.
Suggestions
Share an idea that could help us improve our services, communication, facilities or client experience.
Concerns and Complaints
Let us know when something has not met your expectations or you believe an issue needs to be addressed.
A clear and respectful process
What Happens After You Submit
Here is what you can generally expect when you provide your details and request a response.
1. We receive your feedback
Your submission is received by Connect and directed to the appropriate person for review.
2. We review the information
We consider the details provided and may contact you for further information if you have included your contact details.
3. We respond where requested
If you would like a response and have provided a way to contact you, we will follow up about the concern and any next steps.
If you submit feedback anonymously, we can still review the information, but we will not be able to contact you or provide an individual response.

Feedback and Complaints Form
Share as much or as little information as you feel comfortable providing. The form does not require identifying information, so you may submit feedback anonymously.
If you would like us to contact you, please include your email address or phone number. Completing the optional survey questions also helps us understand your experience and identify areas for improvement.
Privacy and Anonymous Feedback
You can choose whether to provide your name and contact details. Information submitted through this form will be used to review and respond to your feedback and managed in accordance with Connect’s privacy obligations.
Please avoid including unnecessary personal or sensitive information about other people. If your concern is urgent or relates to someone’s immediate safety, contact the appropriate emergency or support service rather than relying on this form.
